If you are signed up to use our Patient Portal, you can use this site for the following (and more):

- To access your medical records, including labs/diagnostic testing, immunization records

- To 'Email' (through the secure system) Dr. Schouten, Sara, or one of our staff with non-           emergency issues, questions, or information

- To request/change/cancel an appointment
*please be aware that charges may apply for cancelling within 24 hours of a visit

You can change your password for this site at any time. To change your password, click HERE

*If you are not currently signed up for the patient portal, but would like to be, please come visit us during our open hours to fill out an email and consent form. You will be given an Account Login and Password at that time.
